These forms help you request a conversation with Macro Practice Partners. They are not clinical intake forms.
What you submit
Your name, email address, phone number, optional SMS consent, preferred contact method, service interest, and consent to be contacted. The practice form also asks for your organization.
Where it goes
FormSubmit processes the form and forwards it to info@macropracticepartners.com. Macro Practice Partners uses these details to respond to your inquiry and discuss the service you selected.
FormSubmit states that it retains submissions in its archive for 30 days. Its spam protection may use cookies or a CAPTCHA. Read the FormSubmit privacy terms and form processing documentation.
Text messaging and consent
SMS consent is optional and separate from consent to respond to your inquiry. If you opt in, Macro Practice Partners may text you about your inquiry, consultation scheduling, appointment coordination, administrative matters, and consulting project updates. Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for help. Consent is not a condition of purchasing services.
We do not sell or share mobile information with third parties or affiliates for marketing or promotional purposes. SMS opt-in data and consent are not shared with third parties, except service providers that support delivery of our text messages. FormSubmit processes submitted form information and sends it to our business mailbox as described above. We use submitted consent information to manage your messaging preferences.

Keep health information out of the form
Use your own contact details. Do not include a child’s name, diagnoses, medical records, or other sensitive health information. If you proceed with behavioral consulting, ask us how to share any necessary information through the appropriate intake process.
Website analytics
We use Google Analytics 4 to understand visits to our public website and interactions such as clicks on our phone and email links. Analytics may use cookies and collect browser, device, page-view, and interaction information. Our tag uses fixed public page addresses and excludes unrecognized URL parameters and editor pages.
We do not send inquiry form contents, names, personal email addresses, phone numbers, or medical information to Analytics. Enhanced measurement can record that a form was started or submitted, using form metadata rather than the values you enter. A form interaction or thank-you page view does not confirm that an inquiry was delivered.
Advertising personalization and Google signals are disabled in our website tag. Learn how Google handles information in its Privacy Policy.
